How does the CDM-HM (Health Management) work?

The DMP module is designed to accommodate the German RSAV and allows you to manage policy holders with the indications diabetes 1 and 2, CHD, asthma, COPD, and breast cancer. Overviews of participant relevant documents with history and support for BVA quality reporting. The module for the care of mentally ill patients significantly reduces administrative workload while providing the best possible care for members who have these indications.
How does the CDM-TM (Telemonitoring) work?
CDM-TM allows you to provide at-home telemedical care to patients with chronic diseases. This results in quicker and more effective intervention on your part, which reduces costly hospital stays or emergency care.
The CDM-TM's integrated Device Connectivity Server facilitates seamless connection, configuration and management of telemedical devices like blood pressure meters and weight scales. Each patient's vital signs and most relevant medical parameters are clearly represented. Modules relating to care for patients with chronic heart failure (CHI) or high blood pressure have already been integrated into CDM-HM.
